What information does WISDOM capture?

The WISDOM Mental Health Information System is composed of the following modules:

Patient Management Module

The Patient Management module captures the patients’ personal details and a range of social and clinical details which include medical and psychiatric histories, risk assessments and alerts.

Every time a patient makes contact with the Mental Health Services, details are recorded under one or more of the following headings: Hospital Activities, Professional/Team Contacts, Day Hospital, Day Centre or Psychiatric Liaison. Each contact type has a set of screens which are designed to capture and display data relevant to that contact.

Letters Module

The WISDOM Letters module allows system users to produce letters using templates and information retrieved from the database. The functionality within this module helps users to create, review and print a range of correspondence easily and efficiently. Each letter is stored in the patient record for future retrieval if required.

Messages Module

The functionality in the Messages module allows users to create and accept referrals and correspond with other authorised WISDOM users.

Scheduler Module

The scheduler is effectively an on-line diary which allows WISDOM users to create, view and update patient appointments.

Reports Module

The Reports module provides the ability to create, edit and print reports based on WISDOM data. An individual set of reports is associated with each type of contact, for example the list associated with professional team contacts includes the following reports: Referrals Received, Contacts, Team Discharges and Standard Activity.

Validation and Quality Assurance

Data validation is performed during data entry to ensure that the patient record is correctly populated, (for example, an admission date cannot be later than the current date). In addition, for each individual patient, WISDOM users can generate a list of the fields which remain empty using the ‘Run Validation’ functionality. This allows WISDOM users, system managers and team leaders to identify patients whose record is incomplete.
